As the number of cancer survivors has increased and as they live longer, it is increasingly important to address long-term side effects, also known as late effects, of cancer and its treatment. A survivorship care plan (SCP) is designed to connect survivors to care and resources they need. SCP’s alert both survivors and clinicians to late effects that require monitoring and screening. They suggest follow-up treatment, wellness and psycho-social support strategies.
