Keeping loved ones in touch with care progress

Our Home Referral Team doesn’t just refer a caregiver and disappear. We stay in constant contact with your loved one via a monthly call and quarterly in-person visit at no additional charge. During this interaction with your loved one, the Referral Team will check for a number of items including obvious changes in personality, temperament, and other home conditions at your request. We then communicate this information to authorized family members or other members of your care team via email or a phone call.  

If, over the course of these calls and visits, any change in the care recipient’s well being is detected, we work with you and your family members to determine if any changes to the Personalized Care Plan is needed.
